Screening Questions—HPPI Outcome Study

Thank you for your interest in participating in the study being conducted by Sociometrics Corporation. First, we’d like to ask you a few simple questions to see if you qualify. Please check the appropriate box for each question.

3.	Do you spend at least 50% of your work time on program planning, implementation, and/or evaluation of HIV/AIDS prevention programs?
4.	Do you have access to a computer with high speed internet access, Windows XP or higher, an available USB port, and speakers?
